Register for COVID-19 vaccine eligibility updates

MHCSI is requesting that UFCW members register their information so that MHCSI can communicate with them as to when the COVID-19 vaccine would be available to them.

This is not a COVID-19 vaccine waiting list, but a site to register your information so that MHSCI can notify you when you are eligible for the vaccine.
